If you are on a Concession Card or a child 15 years, you may be charged at the concession rate.
DVA Gold Card Holders will be continued to be Bulk Billed.
DID NOT ATTEND appointments: If you cannot attend your booked appointment, please cancel or reschedule through HotDoc or by calling the practice. Failure to do so will result in fees.
The doctors will take phone calls relating to you or your family’s health. If seeing patients when you call they may have to call you back. The Practice does not receive emails or SMS’s from patients.
Our Practice has designated times for patients to present and request a repeat prescription. Patients need to present to their regular GP at the allocated time and a script will be generated. If you are unable to come at the special times allocated for your GP, then a short, appointment with your regular GP can be made. If your GP is not available on the day, you will have to make an appointment with another GP in the Practice.
All scripts require an appointment and phone requests are not permitted. Please phone (07) 5482 5899 prior to coming into the Practice, to make sure your regular GP is available.
The Practice will contact you regarding any abnormal results to book a follow up appointment. Please feel free to contact the Practice regarding any concerns you may have regarding results.
Your medical record is a confidential document. It is the policy of this Practice to maintain security of personal health information at all times and to ensure that this information is only available to authorized members of staff.
You may request a copy of your medical records which can be sent to your new practice by contacting the Receptionist of your new practice.
A charge of $30.00 (plus GST) for a full copy of medical records would apply, however a Health Summary is supplied free of charge.

Complaints should be addressed to the Practice Manager either in writing or by phoning and asking to speak to the Practice Manager. The Practice Manager would be happy to discuss any complaints or suggestions face-to-face. If you feel your complaint has not been resolved, you may also like to lodge a formal complaint to:
